for Members who are NOT doing shows

All ILDA Members who do NOT do laser shows must fill out this version of the "Statement of Affirmation" form.

More information about why this form is required for Membership, and who must sign, is on this page. NOTE: If you ARE doing shows now, or plan to do shows in the future, use the form on this page instead. It requires more specific knowledge of laser show safety.

Statement of Affirmation to Safe Laser Shows and the ILDA Code of Ethics For Members who are NOT doing shows
LASER SHOW SAFETY
	I do not operate, produce or present laser shows or displays. Further, I do not plan to operate, produce or present laser shows or displays in the foreseeable future.
	I specifically understand and acknowledge that lasers are inherently hazardous, and that laser safety practices are required whenever a show or display is presented.

ILDA CODE OF ETHICS
	I understand that ILDA Members agree to abide by the Code of Ethics, as a requirement of Membership.
	I have read, and I understand and acknowledge, the Code of Ethics detailed on this webpage and summarized below: -- Members desire to promote cooperative, responsible and ethical application of laser display. -- Members agree to conduct their business in a professional, ethical, safe and courteous manner, and to make a good faith effort to resolve any and all complaints made against them. -- Members agree to respect the goals, desires and objectives of their clients. -- Members agree to observe and obey any and all regulations governing the use of lasers in any country or area in which the member operates. -- No member will knowingly take or receive credit for work done by another member or, through inaction, knowingly allow an incorrect credit to persist. -- Members agree not to make inaccurate or misleading claims in advertising and/or promotion. -- No member will knowingly make inaccurate or slanderous statements about any competitor or through silence lend credibility to statements made by others. -- Members will not misrepresent their capabilities or the capabilities of the medium in general. -- Members agree to keep privileged information obtained by reason of their position in ILDA confidential.
	I specifically understand and acknowledge these two statements from the Code of Ethics: "Members agree to observe and obey any and all regulations governing the use of lasers in any country or area in which the member operates." "Members agree to conduct their business in a professional, ethical, safe and courteous manner... "
